This form automatically appears on the registration page, below all of the registration details. 
All fields are required for registration except for noting a social or corporate sponsor.

If you are unsure of a team name, please use "TBD-" and the captain's initials.
Ex: if Jane Doe is unsure of a team name, she should put, "TBD-JD".  

Do not print and mail this page. Registration is only done online.
